Delta Health Technologies Offers Risk Assessments through Encore

Delta Health Technologies, LLC, a leading provider of information systems and services for home care entities nationwide, is now offering ground-breaking risk assessment functionality as part of its Encore solution for the home healthcare industry.

This reliable, easy to use and highly cost effective tool will enable clinicians to identify how they can mitigate risks on a case by case basis, increasing their ability to develop and deliver the safest plan of care to their patients and to significantly reduce re-hospitalization.

Encore’s user-definable Risk Assessments allow for weighting in order to auto-calculate a low, medium, or high score as well as identify the risks applicable to a home health provider’s unique patient population. This functionality used with Encore’s OASIS Smart Checks gives providers the ability to define and perform consistency checks between OASIS and Risk Assessment questions within one solution, helping to ensure the accuracy of clinical documentation. This provides agencies with the information necessary to implement the steps to mitigate risks — resulting in positive outcomes.

As part of its Quality Improvement initiative, Rockingham Memorial Hospital (RMH) Home Health Care of Harrisonburg, VA completed its integration of Delta’s Risk Assessment tool into its clinical encounter records for Nursing, Physical Therapy and Occupational Therapy clinicians in November 2006. RMH, which has been a Delta client since 1997, has almost 50 clinicians on staff who have utilized the product and have realized significant improvements in time efficiency, allowing them to increase their focus on the patient and decrease the focus on paperwork.

Delta Health Technologies’ Risk Assessment function is available in the most recent version of Encore, which was made available to the market in April 2006.
